• Hospital Admitted
• Reference Details of the Patient given by the hospital
• Address and Contact Details of the Hospital
• Condition of the Patient
• Assistance expected from Sri Lanka Government
• Insurance Information (If available)
• Contact Details of the Next of Kin in Sri Lanka
• Contact Details of the Reporting Person / Organization or persons sponsored for visit
